• 1 Proof of Billing (Bank Statement, Utility Bill, Postal ID, Barangay Clearance, Voter's ID, Envelope with stamp, etc...) Note: Billing must be named under you or else you must get an authorization to the owner of billing that you are really living in the said address.)
• 1x1 Picture
